How to Qualify & Apply for FSW and CIH Waivers

A step-by-step guide to accessing the Family Supports Waiver (FSW) and Community Integration & Habilitation Waiver (CIH) for Intellectual & Developmental Disabilities.

Step 1: Determine Eligibility

To qualify for these specific waivers, an individual generally must meet three criteria:

  1. Diagnosis: You must have a diagnosis of an intellectual or developmental disability (IDD) that began before age 22.
  2. Level of Care: You must require a level of support typical of a supervised setting. Specifically, the individual must have substantial functional limitations in at least three major life areas, such as self-care, learning, mobility, or language.
  3. Financial Eligibility:
    • Under 18: Parental income and assets are NOT counted. Only the child’s income/assets are considered.
    • Over 18: Only the individual’s income and assets are counted (typically requiring SSI eligibility).

Step 2: Submit Your Application

Applications are processed by the Bureau of Disabilities Services (BDS). You can apply online or by phone.
